Your plant alreadyknew this would happen.
Shata Energy OS is Industrial Safety Memory for energy and industrial operations. It brings incidents, near misses, audits, findings, corrective actions, evidence, and asset safety context into one governed operational workspace — so the second failure is not a surprise.
The problem
The same failure, twice.
A seal fails. An investigation is opened. A corrective action is raised, assigned, and left open. Eight months later the same seal fails the same way — and the corrective action from the first event is still sitting there. The knowledge existed. It just was not in front of anyone who could act on it.
Energy OS treats that memory as the product. Every incident, finding, and action is linked to the asset it belongs to, so recurrence is visible at the moment someone opens the record — not after the second release.
Capabilities
One governed operational workspace.
Industrial Safety Memory
The organisation remembers. Repeat failures surface against the asset, the site, and the equipment class — not buried in last year's folder.
Incidents & Near Misses
One record type for what happened and what almost happened, with severity, narrative, root causes, and the assets involved.
Audits & Findings
Audit programmes that produce findings with risk ratings, ownership, and a visible link to the corrective work they triggered.
CAPA / Corrective Actions
Corrective and preventive actions with owners, due dates, overdue ageing, and a closure path that cannot be short-cut.
Asset Safety Memory
Every pump, compressor, and vessel carries its own history — incidents, findings, actions, hazards, and barriers in one view.
Evidence & Documents
Upload the report, the photo set, the certificate. Evidence attaches to the record it proves and stays retrievable.
Knowledge Assistant
Ask in plain language. Every answer cites the records it came from, and abstains when the evidence is not there.
Governed AI Recommendations
Agents propose. They never decide. Prohibited actions are refused by policy and recorded as refused.
Full Manual Workflows
Nothing requires AI. Every workflow can be driven entirely by people, with the same controls and the same record.
Audit Trail
An append-only ledger of state transitions, decisions, and who made them — built for the auditor's question, not the dashboard.
Executive Risk Visibility
Open findings, high-risk exposure, overdue actions, recurring failures, and assets with repeat incidents on one screen.
Governance
An agent may propose. Only a named human may close.
Safety-critical actions are on a prohibition list that the AI layer cannot execute. When an agent attempts one, the attempt is refused by policy and written to the ledger as refused — visible to you, not hidden.
01
Server-side authorization
Role and site scope resolved on every request
02
State machines
Only legal transitions are offered, and re-validated on submit
03
Segregation of duties
The closer must not be the owner
04
Evidence before closure
No verification evidence, no close
05
Append-only ledger
Decisions are recorded, never rewritten
Try it yourself. No call required.
The demo runs on a seeded synthetic dataset — invented sites, assets, and incidents. It is not customer data, and it is not connected to any real plant. Sign in as any of the seeded roles to see how the system behaves for that role.
- 1Open the demo and pick a synthetic role on the sign-in screen — no credentials to request.
- 2Open the pump with two seal failures in eight months and read its Safety Memory.
- 3Ask the Knowledge Assistant whether it has happened before, and check the citations.
- 4Watch an agent be refused permission to close a corrective action.
- 5Close that same action yourself, with evidence and named attribution.
Where it stands
Energy OS is a pilot candidate, released as a controlled demo. The workflows, governance controls, and audit trail shown in the demo are real and running. Enterprise identity (Entra ID / Okta) replaces the demo sign-in for a production pilot, and ERP integration is scoped per engagement rather than assumed. If you are evaluating it for a real site, request a pilot and we will walk through the boundaries with you honestly.
Start a pilot
Bring your site's safety memory into one place.
Tell us about your operation — sites, assets, and how incidents and corrective actions are handled today. We will scope a pilot around what you already do.
